Any page that requires authentication redirects me to nothing (e.g. /cgi-bin/, which is forbidden). What's going on?
Upgrade your CGI.pm! Using something like 2.36 gives this behaviour, upgrading to 2.56 or later fixes it.
If, upgrading CGI.pm, it still fails because the older version is still being found, change the first line of fom.pl to:

        #!/usr/local/bin/perl5 -wT -I~/user_perl

This happened to me as I did not have root access and could not de-install the older version in /usr/local/perl5.

I have the same problem. How do I upgrade to the latest cgi.pm?
